Record keeping in the psychosocial practice

Record keeping is the tracking of what you do and agree with a client. In QiC’s psychosocial branch fixed places are set up for it, so you do not have to work out each time where something belongs.

What you record

Intake, consent, the plan and progress. Questionnaires can also be part of it, if you set them up.

Because those parts are in the record, you can later show what was agreed and when it was recorded.

No automatic formulation

QiC helps you organise complaints and criteria. You write the formulation yourself, the software does not complete it.

That is deliberate. A formulation is a professional judgement, and it belongs to the practitioner.

Examples

An example questionnaire is an example and not an official standard. You can use it as a starting point and adapt it to your own practice.

That way you do not have to start with an empty list, and you stay in charge of what goes into it.
